Job Description

Job Summary: The Pharmacist reviews and interprets physician orders in accordance with evidence based best practice guidelines and literature for dispensing to caregivers for administration. Manages medication orders to be in compliance with the "Formulary"; Compound medications in accordance with Good Manufacturing Practices (GMP) and Food and Drug Administration (FDA) or United States Pharmacopeia (USP) standards. Distributes and dispenses controlled substances with security and documentation per Drug Enforcement Agency (DEA) regulations. Supervises pharmacy technicians in drug distribution and compounding tasks. Provides drug information to professional staff. Counsels patients and family members regarding medication use as appropriate. Assures drug use in different patient populations is safe and effective, i.e. geriatric, pediatric, neonatal patients. Possesses competence in clinical medication management (pharmacokinetics, renal dosing, clinical protocol execution for specific patients, etc), order entry and verification computer systems, California and Federal Pharmacy laws. Participates in adverse drug reaction reporting program. Performs other duties as needed.

Education and Experience: Graduate of an accredited school of pharmacy required. Two (2) years of experience in an acute care hospital pharmacy or completion of a two-year pharmacy residency program required.

Knowledge and Skills: Knowledge of preparation and dispensing of pharmaceuticals pursuant to interpretation of a licensed physician's orders. Knowledgeable of and complies with laws, rules and regulations that govern the practice of pharmacy. Competency in all aspects of drug therapy including factors related to patients of all age groups including neonates, pediatrics, adults and geriatrics. Able to read; write legibly; speak in English (and Spanish preferred) with professional quality; use computer and software programs necessary to the position (i.e. Microsoft Windows, Office including Word, Excel and PowerPoint, internet, timekeeping, e-mail); operate and troubleshoot basic office equipment required for the position. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place and patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.

Licensures and Certifications: Current license by the California State Board of Pharmacy required. Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications from the American Heart Association preferred.
